Hazard signCeiling suspended devices
are failing

The Medicines and Healthcare Products Regulatory Agency (MHRA) has issued an alert on devices in cardiac catheterisation and interventional radiology laboratories, including equipment suspended from the ceiling.

The MHRA has received reports of adverse incidents where devices have not been maintained, leading to failure. In some cases, there has been no maintenance undertaken for several years. Suspension arms supporting ceiling-suspended lead acrylic screens, theatre lamps and power injectors can come loose, or fail.

One example was a ceiling-suspended radiation screen where maintenance on the supporting arm had been overlooked. This may have contributed to the failure of a knuckle joint which caused the arm to collapse.
